Roles of the Gallbladder & Bile Ducts

The liver produces bile, and
the gallbladder stores it and releases concentrated bile when needed.
Bile ducts carry bile from the liver and gallbladder to the small intestine,
playing an important role in fat digestion and waste excretion.

When the bile duct or gallbladder is blocked by stones or tumors, bile cannot drain properly,
leading to symptoms such as jaundice, fever, and abdominal pain.
In such cases, PTBD (Percutaneous Transhepatic Biliary Drainage) or PTGBD (Percutaneous Transhepatic Gallbladder Drainage)
can drain bile externally to relieve symptoms and prevent infection.

When Drainage Is Needed

Bile duct stricture or obstruction
Need for preoperative biliary decompression
High-risk acute cholecystitis
Not a candidate for surgical treatment
Persistent gallbladder distension and pain
PTBD

Percutaneous Transhepatic Biliary Drainage

Indications Biliary obstruction/stricture, cholangitis or jaundice due to bile stasis

Procedure Catheter inserted into the bile duct through the liver

Feature Intrahepatic access favors long-term drainage maintenance

PTGBD

Percutaneous Transhepatic Gallbladder Drainage

Indications Acute cholecystitis; patients unfit for general anesthesia or surgery

Procedure Direct catheter placement into the gallbladder via the liver

Feature Direct gallbladder drainage enables early relief of pain and fever

It Can Be Painful, But It’s Essential

It lowers inflammation and pressure in the bile duct/gallbladder,
restores bile outflow, and
effectively resolves problems caused by cholestasis.
